Reach Healthcare Services employs a talented group of nurses and experienced caregivers who are highly trained in delivering every form of home health care.  This caring group of individuals are compassionately serving our patients needs either in the office by scheduling supply deliveries and politely providing health care solutions over the phone or visiting patient’s homes to administer quality care in a friendly way.

Tameria Kelly-Peña, RN

I’m the Director of Clinical Services at Reach Healthcare Services for the Licensed and Certified Division and joined their team August 2015. I’ve been working in home healthcare since February 2006 and held a title of RN case manager/supervisory nurse for 6 months until I was promoted to become the Director of Nursing.  I also have experience as an Administrator for Jaeg Bright Medical Services for 3 years.  I am bilingual and able to speak, read and write Spanish. Prior to working in home care, I was the charge nurse at Bayshore Medical Center for the Pediatric unit and have experience in Newborn nursery, Post partum and Med-Surg floor as a floater. I love doing home healthcare and being the supervising nurse, especially interacting with patients. All my patients love me because I treat them like family. I am married and a mother of 5 beautiful children and adopted a 6th child 6 years ago. I am happy to be a part of Reach.

Lily Esparza, BSN, RN

I am Hiliana Esparza-Miller, the Clinical Director for all services provided by Reach Healthcare Services, but my family, friends and colleagues call me Lily.  I am a daughter, sister, wife, mother, and friend. I have had the honor of serving my country in the United States Marine Corps and serving patients from a 28 week premature baby to a 102 year old. I have worked the hospital floor at night, infused a Pediatric patient at home in the midst of a hurricane evacuation, and directed the clinical care of a multi-million dollar home health agency. I chose the nursing profession after my tour of duty due to a need to provide for my family. However, this profession has rewarded me time and time again in more profound ways than just monetary. Serving my patients and their families as they recover at home or settle in to a chronic diagnosis has taught me life lessons and fuels my desire to advocate for them in every way possible. Joining Reach Healthcare Services in 2014 gave me the platform to deliver holistic patient care and continue growing both as a nurse and an individual. If the need arises for care, I hope to have the privilege of serving you and your family.

Christina Sanchez

I have worked in Home Healthcare for Fourteen years and this year will be five years I have been with Reach Healthcare Services. I have had the pleasure of working in the Skilled Nursing Department processing referrals and scheduling for our nurses. I am now in the Personal Assistance Services and Private Duty Nursing department doing scheduling and processing payroll for PAS and PDN. I really enjoy working for Reach and love how great the staff works together for our patients needs. I love knowing that our patients are deeply cared for and they have the care they deserve. 

Carol Rahe

Carol Rahe I have worked in Home Health Care for twenty-eight years, twenty-two continuous years for REACH Healthcare. I have had several job responsibilities throughout my career, Patient Care Coordination, Medicare Billing, Processing Referrals & Paperwork. For the last five years I have worked as a Reach Out Advocate working with Private Duty Nursing and Caregiver Service clients & Marketing with our referral sources. I enjoy speaking with our clients, families and referral sources. My hobbies are traveling, enjoying nature, walking and spending time with friends and family.

As Carol has mentioned she has worn many hats over her long career at out agency. She has long been our agency historian and can remember patients, staff and events that most of us forget. She has a unique ability to connect with people and possesses the valuable skill of listening and showing you that you are important enough for her to hear you. She is kind, thoughtful and is a presence that all of us have come to depend upon. She takes on the responsibility of her tasks related to patients as if those patients were her own family.

Christy Dobson

Christy Dobson I have been in homecare for 15 years. I have been with Reach Healthcare Services for 12 ½ years. I have had multiple job duties through the years with this company from working in IV, scheduling for IV, HHA, PDN, and PAS patients, payroll, working referrals or other clerical functions. I now am the Clerical Supervisor for nursing department and Supervisor for PAS staff. One of the reasons I love Reach Healthcare Services is the fact that I am recognized. I have the opportunity to know the CEO and Executive Director of the company and they know me by name. I also like the caring people that I work with. Reach Healthcare Services have great nurses and staff. I would not trade it for anything.

Christy has become an integral part of our agency. She keeps all of us in line. She is an incredible multi-tasker and adapts to changes flawlessly. She cares deeply for our patients as well as our staff. Over the years she has grown with the Department and is an incredible resource. She is dedicated, dependable, adaptable and invaluable.

Sally Barnes, LVN

Sally BarnesSally is one of our field LVN’s. She sees patients under the direction of our Field Patient Care Specialists and works primarily in the South and East parts of Houston and surrounding areas. Sally has been with our agency for 9 years and is loved by all her patients as well as the staff.

 

 

Andrea Dancy, MSW

Andrea is our Social Worker. She has specialized experience to assist our patients with community resources available. She also works with a local Dialysis center and has a unique understanding of Medicare, Medicaid and governmental assistance.
